what do you think i should do?

ideally i'd change it but do i "need" to?

Its only surface rust imo. A bit of sand paper and some paint. Job jobbed.

Sand it down, bit of silver paint and she's mint some cars drip water from washers or old seals etc onto engine components and rust them out :-(

No disrespect but it looks like your engine bay has been submerged in water at some point as there seems a lot of corrosion on everything !! A really good clean though should sort it out and as others have said sand it down and repaint it silver.
