Default ford focus dtci/ cold starting probs

hi new to the thread
can anyone help or had the same probs ford focus 2002 tdci when cold takes 10 - 20 secs to fire up and the engine shakes like mad and loud knocking from drivers side ? i belive the shaking & knocking is the dual mass fly wheel . i"ve changed the glow plugs / fuel filter. ps the car starts fine when warm thanks
