possibly duff thermostat if the gauge reads low, if not then i'd say a air lock in the cooling system, just take the coolant cap off and leave it running. you should see air bubbles coming through..

blockage probably in the matrix or air.
either run with cap off, park up hill with cap off (as this will help air get out) or fit a hose pipe in the heater matrix pipe and let some good old mains water pressure clear the shit out of it.

Took it to a garage in the end ..Heater matrix blocked he tried flushing it but to no avail..Said its a nightmare to do as he has to loosen the dashboard as it wont come out no room .The new one is slightly modified apparently ..This one has flat pipes the new one has round..Be a fortune I reckon as matrix is Ł50

quiet a common problem this, i had the same problem with my escort td. i managed to flush mine with the house pipe and a air line, fill it with water and then force the water out with compressed air, ideally in its backwards flow dirrection. mind you, ts still not 100% but good enouph now.
and as to labour, your garrage is about right with the quote as the dash has to come appart and possibly out. a nasty job to do really.

I've dropped some costic sodium tabs into the header tank and drive car around for a day and then flushed out with a power hose as mentioned above and after a while the water flowed through matrix and heating worked finee!
regarding labour if he's/she abit handy with the tools can have it done 2-4 hours tbh!

You say the heater gets warm when the car is idling. I would still be changing the thermostat just to remove it from the equation, as its a cheap and simple thing to do, and may well fix it.
