Normal reason for failure is teh head bolts coming loose in my experience.

Did any feel a little slack upon removal?

why do they "come loose" Stu? not torqued up properly in the first place? or just lose tension with age?

Using old head bolts will cause them to come loose!
The bolts are made to stretch so after fitting and then the engine getting warm the head will expand and the stretch will compensate for that expansion.

If you use old bolts then they would have already stretched and so the head will just pull them thus loosening them from the block once the engine has cooled!

Was that block 'properly' cleaned-up before fitting that cometic gasket? What is left on the top of the block looks like very old debris from a previous standard type gasket to me but it could just be the shonky camera/pics
