if you look the bit that is broken off is the same shape as a pad

I can't imagine what kind of heavy braking you would need to do that to a disc.

Surely the Master Cylinder/Brake Hose/Brake Pad/Drivers leg would fail before taking a chunk that size out of a disc?

maybe you kept your foot on the brake after a heavy braking session previously (when hot) and the damage was done then?

looks scary either way
