What sort of construction is your house?
It's really quite straight forward - chase/pull cables through the walls/behind skirtings, get a rack or brackets for boxes and connect up as normal.
HDMI leads can happily run for 10-15m at 1080p, any further and quality of cable becomes a significant factor due to capacitance. All other formats will be fine.
As far as remotes go, there are numerous products out there to transmit and repeat the signal if you want boxes out of site. Some are wired some are not. Sky wise, you can use a triax link on the RF2 output (1 extra co-ax), or get a multi-output repeater and use it for DVD and Sky.

The PJ is a Optima HD65, pic was not a very good one, here is another to try and show the quality, I can assure you it is very good (unlike my camera skills!)

As for the height, I have kids hence need the screen high, the screen is tilted and the sofa is a big "lay back" type, see pic. Had it like this for 2 years now and nobody ever notices the height when watching

In a bid to child proof the house and have nothing showing I also have the whole system based in the attic running Windows Media Centre and extenders to all the rooms, this gives me access to 250 odd DVDs, I-Tunes collection, Familly pics and videos etc all stored on the Hardrives and accessed by a nice simple interface on any tv in the house (Boys and there toys!)

I really hate high TV's. I always have my TV's so the centre of the screen is more or less at eye height in a normal sitting position, which as Fil says, is about 1m from the floor....
Anyway, back to the OP, mount your screen on the wall at your desired height, cahse out the wall for cables (remember to include one for each input your TV has for future proofing). As for where to put the source gear, don't put it in a rack near the sofa if you have any noisy gear (such as a PS3 with fan noise etc) I'd put it in a small rack in the corner of the room personally

mine, wish i'd done it years ago. it's not as high as it looks, as the top of the picture is a picture rail about 18" from the ceiling.
i put the satellite box in the space that used to be a fireplace.
