Default replace a lost log book

can anyone point me in the right direction ,my friend has bought a volvo truck on a g plate to restore,he has the reg and the chasis number ,how would he go about getting a replacement logbook[presumed lost] from the uk authorities

download one of them, fill it in, send it off with the fee and bobs your uncle.
