Who makes custom exhaust manifolds?
As title need to get one for my 200sx (ca18det) as the quality of the e-bay manifolds are pretty poor quality.
Any recommendations on who to use and roughly how much?

The trouble is, you can buy at least 5 of the cheap ones for the price of one custom made one! 
Has Bren not done a decent one yet? I know he has the 'MOFO' manifold as he calls it for the S14, with 3mm wall thickness, but not sure about the S13.
