I watched that program........was shocked at the attitude of Jarvis (the line maintainence contractors), not allowing it's employees to answer ANY questions without a company solicitor being present
Also, with the points slider bar lock nuts working loose and causing that accident (8 fatalaties and many horific injuries), why wer'nt they using Nylock nuts throughout the network - a proven solution where vibration would be a problem
And then, towards the end of the program seeing video proof that the Jarvis staff (or sub-contractors) blatently did not know how to use/tighten simple lock nuts

The program certainly made interesting viewing.
While at work today, I had a look at a couple of sets of points on a mainline. The stretcher bars had not been replaced with the new "cam locking" nuts - still had the old type locking nuts on them
Altho, Jarvis no longer do ANY maintenence on the railway - its all done "in house" by Network Rail

When I worked in secuirty, I was contracted to work security when they installed the new bridge at the Potters Bar station. Was a spectacular site seeing them actually lift and install the already put together bridge 
Didn't see the sho tho.....
