More likely a lubrication problem, or air pocket and hot spot around no 4 cyclinder.

compare 4th injector with others

it can give less fuel and even when AFR (average) is good it can be too lean on 4th cylinder

pinkin - detonations
can be result of failed injector

There has to be something wrong with your fuel system then

injector/injector pipe/metering head, clearly under fuelling on cylinder number 4

i just had a quick look at the fourth injector the seal is fucked and the clamp that holds the injector in place is bent upwards so the injector moves in & out between 5-10mm could that be the prob?

yes

Ive got a set of injector seals and collets if you want to buy some, the collets alone are in excess of £3.50 +VAT from Ford

Clamps are NLA but you can buy stainless ones from auto specialists

check your voltage at the pump as old wiring can cause LOW volts at pump = less fuel=lean= melted piston=£££££'s or as above injector fault, have the fuel pressure checked by a pro before u large it again
