The clutch plate is what 'transmits' the drive to driven wheels hence the name via the drive train.
The plate is lined with a friction material (Used to be asbestos) when you release the pedal the plate is sandwiched between the flywheel and the pressure plate, in the centre of the clutch plate is a whole with splines, these splines connect onto the input shaft of the gearbox and so the power is transmitted. When you disengage the clutch the disc is seperated from the flywheel by the release bearing pushing against fingers on the pressure plate and so the clutch disengaged.
