Flywheel coming off and through the bulkhead ??
I remember seeing a link to a car that its flywheel came off and went straight through the gearbox and bulkhead.... anyone got the link ??

if you don't put the bolts in, this can happen to any engine
but i've seen a few where the flywheels have come though the bellhousing, mainly on these newer dual mass flywheel contraptions where the bits simply slipt apart

Lightened flywheels, cheap crap ones, dodgy cheap crap clutches can all explode, with pretty nasty consequences.
IMO, if quality parts are used, and correctly fitted, it simply will not happen.
Although most racing in the US requires SFI approved rotating parts, that have been proven to stay together. Go below certain ET's, and you need a fully enclosed steel bellhousing or bellhousing shield to prevent any failures posing a danger.
The most recent pic I seen, was of a Skyline....probably with a cheapy ebay clutch/flywheel...or else fitted by a dopey git, or both.
