The are a few reasons I can see that happening.... all IMHO but I recon any of them could cause it.

1) crap oil pressure to the head, either a shit pump, incorrect grade oil, or blocked oil feeds?
2) New followers on an old fucked cam, a pitted cam will tear new followers a new arsehole in a matter of hours.
3) new cam and followers not run in using cam lube/ allowed to idle and the surfaces have torn each other apart.
4) bent camshaft/incorrectly torqued down/ pulled or slack cam studs allowing the cam to run off centre.
5)cheap ass crap quality followers

of all the above, my money would be on a lubrication problem or a bent cam... what are the journals like, are they ok, pics not very clear, and some pics of the cam cams and the head journals would be useful as well.

Head is new,
Block is new,
Cams are new,
New INA followers,
New oil pump and drive,
Fresh Silkolene 10/50,
Cam studs were torqued and locktited down,
Unsure if cam lube was used but I am pretty sure it was.

There was loads of oil up at the top of the engine too. It's only happened to 2 followers and 2 lobes out of 16.

Swarf blocked a couple of your oil feeds to head. Or cam hasnt been "run in" IE: Run at 2000rpm for 20 mins on first start up IMO

If so are you sure they were genuine and not the dodgy copies that are doing the rounds ?......same thing happened to a mate of mine and when we had them checked the hardening was of very poor quality....

i brought a piper cam kit brand new and although it was on a cvh enine looked the same mess and it was down to the lifters hadent been hardened properly which in only a few miles wore the bottoms of the lifters away and the cam lobes.. they paid a garage to fit another kit from another batch and all was fine..
