What would cause a hole to blow through gearbox?

Got to Derby saturday night after taking fiesta on track at combe and i was cruising about 30/35mph and suddenly bang,a hole blew through box near speedo cable
Thats 2 dead RS's within a week

Oil running hot inside causing one of the cogs or bearings to weaken - clutching at straws there though as I don't know too much about gearboxes! (apart from what they do!).

Is the breather blocked causing a pressure build-up on the inside ?? Happened to me at Llandow last year but it blew out the small core plug inside the bellhousing.
Hope the motor sounded well onto the A5
