Well I was adding a couple of new Fibre channel switches to a existing fabric, I have done this so many times and as a project team was pushing me I did not put it though change controll, well sort off, well the fabric went down.

the whole finance systems crashed for the whole as Daily mail and all reginal news papers, all the zone configs went AWOL for 2 mins
