The upper face of the solid mount where it sits on the chassis needs to be parallel to the face of the beam where the two bolts attach the plate.

If it is not then the big bolt twists the plate around its hole in the plate to compensate.

Its clear from the photo that is exactly what has happened in order to take up the slack.
