I've heard (and this may or may not be true) that the reason for a few engine failures on the few off the production line have been related to a bearing in the engine and this is whats caused the engine to fail. From what i've heard the bearing supplier had changed from the ones that were fitted to the test/prototype (no problem with them) and the production ones and this could be the explanation to the delay of the new FRS from production lines.
