This happened to me with my old router, started to have faults like you mentioned above and I went through everything to no avail. I am a qualified network engineer and know what I am doing, so I knew all settings etc.. where fine. I bought a new router and this cured the problem.
Recently I updated the firmware on my router and it stopped the Xbox wireless adapter from working, due to new security settings/protocols so I had to load the older firmware and everything was good again. Never leave any items set to auto update the firmware as sometime you cant roll it back.
