According to the ford service bulletin, you wil most probably need the modified bottom pulley, tensioner and belt replacing
thats where my money is too, although quite a lot of high milers were duel mass flywheel.
you need to hear the sound really to make the best guess
can best describe it as a CVH with little or no oil in it on start up , or maybe a kent type with unadjusted tappets , mixed with a rattle
