Default CVH Cam Timing

Technical question. Hopefully someone can help me .
In the process of taking head of my 1.6 CVH engine. Manifolds , covers etc are all off, timing belt still on.
The original ford cam sprocket has been removed and there is an adjustable sprocket which presumably suggests a performance cam shaft has been fitted.
The issue is timing marks. There is a punched dot on the face of the sprocket and this does line up with the mark on the head when the crank is at TDC. However the rotar arm is pointing at piston 4 and not 1.
One of the adjustable screws on the sprocket has a blob of paint on it and when the crank is at TDC and rotar at piston 1 the back of the screw does line up with the timing mark on the head.
Don't want to take the belt off until I know what us going on.
Sorry if I have not explained this too well!
Any thoughts on what's going on??
