I've been using the dual pads lately

Gives you something to hold on to and you can also flip it over pans use the white side if there is a deeper scratch or stubborn mark.

Foam applicators can be used again and again. Mine go in the washing machine at 40 degrees. I guessing yours either wasn't dried properly or got damp or something during strorge and disintegrated. Wash, dry on radiator and they will last for ages.
