Are the Piper pulleys known for this? I guess the alloy could wear away or possibly an issue with the teeth.

OTOH what belt was used on the car? On some cars (a previous car included) Contitech belts with their rounded teeth had a reputation of slipping as well as stretching in some cases. A number of belts (Dayco and Gates being two) appear not to have rounded teeth.
