This breather I am showing you now only has 4 also mate, This has 3 on top, and one underneath, compared to having 2 on top and 2 underneath.
So if yours have got 4, then it should be the same as this. Rocker cover, catch tank, Block adapter (remove kidney dish), and turbo side block.
The best thing to do is to mount it roughly where you want, and then decide which port will match up with the others. It doesn't matter what goes to what port, but naturally once you have it mounted the pipework is best fitted in which ever way seems the most logical
Personally, if i had 2 inlets on the top, i would make the top 2 go to the rocker cover and the catch tank, and then leave the bottom 2 for the 2 bottom blocks. I had to loop round the back of the engine with some pipe (which i heat guarded), in order to get to the turbo side.
You dont have to have anything connected to anything on the inlet, just block that.
