If its a 3 port:
1 large pipe at the top of the breather goes to the location of the original breather (which you remove) under the inlet man.
The other large pipe goes to the other side of the block under the turbo. You need the adaptor that Bailey supply.
The small outlet at the bottom of the breather goes to the sump below oil level.
