I've got a pair of bells for 356mm AP discs (12 bolt, can't remember the PCD, but it's around 9-9.5"ish).

They've got an offset, from the hub mounting face, to the disc mounting face, of 24mm, IIRC (I'll be able to measure them up at the weekend if you're interested).
