The most likely cause is becuase you are running rich all the time. O2 sensor do not like it, their job is to stop it happening. However, if the mapping is too far out the O2 sensor can not do it's job properly.
It runs closed loop witrh CO of 0.7% at idle when the sensor is working so its not running rich
That is at idle, how long per journey to you stay at idle?
not very long, the last sensor lasted less than 100miles!
