Default cold starting

what would cause very lumpy idle and poor running when the engine is cold?? would poorly adjusted cam, or adjustable followers?

compression test is normal once warm, not sure if you can do one and get any suitable results when cold??

as soon as car is warm its fine, only cold when it sounds crap.

i'm thinking something is wrong with the follower gap maybe?

any thoughts welcome.
