you just put a spanner into the hole in thesection where the captive nuts are and hold it there surely
unless you can't?
thats what i did when they snapped off in my mates car

as for getting them out, what you really need is a socket big enough to compress the outer bit of the lip of the bush as it comes out of the top and then push it through
