saw a similar one about 10 years ago - sheet metal on back of truck, not secured properly, truck driver did emergancy brake application after someone pulled out infront of him at a junction, truck stopped, sheet metal didnt, sliced cab in half killing the driver.

that was someone in the midlands, cant remember where exactley, was only 11 at the time
