Default Fault Code Results

My car is cutting out after either a bit of a strenuous blat or when stuck in straffic. Both symptoms point to the car being hot and, so I ran my Gunson Fault Code Reader on the ECU. These are the error codes it came up with: -

24 - Depending on fitted sensor: Air Change Temperature sensor (ACT) or Vane Air Temperatures sensor (VAT)

35 - Throttle Position Sensor (TPS) voltage too low

37 - Manifold Absolute Pressure (MAP) value too low

59 - CO% adjustment potentiometer (REMCO)

The car will idle happily until it's warmed up, then it will start hiccup ever so slightly, every ten seconds or so.

Anyone want to spread their wealth of knowledge on any possible ideas to fix them? And what the cause of my problem might be

I assume 24 and 59 means that the FCR didn't pick them up during the test.
